According to journalist Sacha Tavolieri and reported by Voetbalkrant, the Belgian winger has had enough of his miserable time at the Bernabeu.
He is not central to Carlo Ancelotti’s plans and is ready to move on.
The reports add Real Madrid would be open to selling the £416,000-a-week forward for just £28million as they prepare to cut their losses.
Hazard, 31, has played just 59 times since his £88million transfer from the Blues in 2019, scoring five.
This season, his tally is zero goals from 16 appearances totalling 722 minutes of football.
He was an unused substitute in Sunday’s 2-0 Spanish Super Cup win over Athletic Bilbao.

And he cut a despondent figure amid the celebrations, not even smiling as the team posed with the trophy.
It is also suggested injury-plagued Hazard needs surgery to fix his latest problem.
But Real Madrid president Florentino Perez allegedly blocked the treatment to help guarantee a transfer.
Hazard spent seven years at Chelsea, scoring 110 goals to place him ninth on the all-time list at Stamford Bridge.
Reports in December suggested he could head back to West London at the end of the season once his fifth child has been born.
But Real may insist on including Reece James in any deal due to their interest in the England right-back.
